When a new patient comes into the clinic, we gather as much information as possible about their health history in order to ensure the absolute best treatment. To optimize your time in the clinic, please download the New-Patient Forms (click link below) and print them. Then fill them out and bring them with you on your first visit.
If you are coming in for a Meridian Stress Assessment (MSA), be sure to download and read the Patient Preparation Form as well.
